Health & Nutrition Community Mobilizer Taiz Location: Maqbanah District, Taiz Number of Service Providers: 2 Contract Type: Third-Party Service Contract Sector: Health & Nutrition Activities: Health Activity 1.1.2; Nutrition Activities 1.1.2 and 1.2.1 Job Summary: The Health & Nutrition Community Mobilizer will support community outreach and mobilization activities in Maqbanah District to increase awareness and access to available primary healthcare, nutrition screening, and IYCF services. The service provider will engage with communities, caregivers, pregnant and lactating women, children, and other vulnerable individuals, facilitate appropriate referrals, disseminate key Health and Nutrition messages, and identify barriers affecting access to services. The service provider will coordinate closely with the Health & Nutrition Officer, supported health facilities, and relevant community structures, maintain accurate records of outreach and referrals, follow up on identified cases as required, and submit verified monthly records and deliverables in accordance with the service contract. Key Responsibilities: - Conduct regular community outreach and mobilization activities in targeted communities in accordance with the approved monthly workplan. - Promote awareness and appropriate use of available Health and Nutrition services at the project's supported health facilities. - Disseminate approved messages related to primary healthcare, maternal and newborn health, child health, immunization, nutrition screening, IYCF, and other project-supported services, as applicable. - Identify children, pregnant and lactating women, and other vulnerable individuals who may require Health or Nutrition services and facilitate referral to the appropriate service point. - Support caregivers and community members to understand where, when, and how to access available services. - Follow up on referrals, where appropriate, and report barriers affecting referral completion. - Identify and report misinformation, misconceptions, access barriers, service gaps, and other community concerns to the Health & Nutrition Officer. - Inform community members about available feedback and complaint mechanisms, including the project hotline/CFM, and explain how these channels can be accessed. - Coordinate with the Health & Nutrition Officer supported health facilities, community structures, and relevant volunteers to ensure consistent outreach and referral messaging. - Maintain accurate records of outreach sessions, participants reached, key messages delivered, referrals facilitated, follow-up actions, and issues identified. - Submit complete and timely monthly records and agreed service deliverables for verification. - Maintain confidentiality and comply with safeguarding, PSEA, protection, accountability, and organizational procedures. - Perform other directly related outreach and mobilization tasks assigned within the scope of the service contract. Performance and Service Deliverables Monthly performance will be assessed against the approved workplan and verified records, including: - Outreach and mobilization activities conducted as planned. - Community members reached, disaggregated where applicable by sex and age. - Approved Health and Nutrition messages disseminated. - Referrals identified and facilitated, with appropriate documentation. - Referral follow-up undertaken where required. - Community barriers, concerns, and access issues identified and reported. - Information on CFM/hotline and available referral channels provided to communities. - Complete, accurate, and timely monthly activity and referral records. Required Qualifications and Experience: - Secondary education is required; a diploma or relevant training in Health, Nutrition, Community Development, Social Work, or a related field is an advantage. - Previous experience in community mobilization, health/nutrition awareness, outreach, referrals, or humanitarian/community-based activities. - Good knowledge of the local community and context in Maqbanah District. - Good communication and interpersonal skills, with the ability to engage respectfully with community members and vulnerable groups. - Ability to conduct community outreach activities and communicate Health and Nutrition messages clearly. - Ability to identify and report community-level barriers and service access issues. - Ability to maintain accurate activity and referral records and submit timely reports. - Commitment to confidentiality, safeguarding, accountability, PSEA, and protection principles. - Fluency in Arabic; knowledge of English is an advantage but not essential. Key Deliverables / Verification: Monthly payment will be linked to verified deliverables, including: - Number and records of community outreach and mobilization activities conducted. - Number and profile of community members reached. - Health and Nutrition messages disseminated. - Referrals facilitated and documented. - Follow-up conducted with referred individuals/caregivers, as required. - Key barriers and community concerns identified and reported. - Information provided on feedback and complaint channels. - Complete and accurate monthly activity records submitted on time.
